SCREENING

Marbella CardiologyGeneral SCREENING of patients in the general population to identify those of having coronary artery disease (CAD). This should comprise the following:
General MD’s for physical exam and history taking of traditional risk factors such as smoking, hypertension, diabetes, dyslipidemias
EBCT for Coronary calcification score
Abdominal CT for visceral fat quantification
Laboratory testing to include a fasting lipid profile, fasting glucose (insulin resistant), C-reactive protein, and non-traditional CAD risk factors such as Hyperhomocystememia.
EKG
Exercise stress-testing

RISK FACTOR MODIFICATION

Marbella CardiologyOf those patients that have any abnormal results of those mentioned above they are de facto identified as being at risk for CAD. Therefore the second base to this structure should be based on RISK FACTOR MODIFICATION. This should include:
Hypertension clinics and evaluation using echocardiography for the diagnosis of Left Ventricular Hypertrophy.

Lipid clinics for evaluation and management of Dyslipidemias using standardized treatment protocols
Smoking cessation clinics
Diabetic clinics to not only evaluate and manage patients with frank clinical diabetes but those who have the so-called “insulin-resistance syndrome”
Weight loss evaluation and management, in close collaboration with dietitians and endocrinologists.

TREATMENT

The third base includes TREATMENT of patients who have active CAD or those with a previous history of a myocardial infarction. This needs to be done by a trained cardiologist and entails
Treatment of angina and/or left ventricular dysfunction
Obviously coronary angiography + percutaneous coronary interventions (PCI/ Angioplasty)
Management of valvular heart disease.
Implantation of biventricular pacemakers and automatic implantable defibrillators.

CARDIAC REHABILITATION

Last but not least the fourth base of this structure should be based on CARDIAC REHABILITATION
Includes supervised gradated exercise regimens after myocardial infarction.
A gym-like site where patients can exercise and meet with appropriate professionals such as dietitians, psychologists if needed.
